Our practice operates in English. Patients who prefer another language may bring an adult interpreter of their choosing, as they deem necessary. We do not provide in-house interpreter services, but we’re happy to pause or reschedule if communication isn’t clear

All patients in this office schedule on a self-pay basis and so they all have the right to receive a Good Faith Estimate of expected charges for healthcare services. Med Exam for Class A/B/C Licenses (Federal Exam Standard)

The FMCSA DOT physical exam is for Class A/B/C license drivers required to meet the Federal physical standards (reported on Form MCSA-5876 Medical Examiner's Certificate. This is the correct exam for Class A or B commercial license drivers or for Class C drivers who must meet the federal physical standard for a state endorsement (e.g., ambulance drivers, first responders...). Drivers will also be given a copy of Form MCSA-5875 Medical Examination Report Form.

Med Exam for Class C RV drivers or Instructors (California Std. Form DL546A)

This is for state required certification of NON-COMMERCIAL drivers, including 1) driving instructors or 2) those driving a House Car 40 Feet+.
